Biofilm management in restorative dentistry
Advances in the understanding of dental caries have shifted restorative dentistry toward biologically informed strategies, with Minimal Intervention Dentistry (MI) becoming a key guiding philosophy. MI focuses on two main components: effective management of the cariogenic biofilm and the use of minimally invasive restorative techniques, particularly when addressing the infected layer of advanced lesions. Developments in bioactive materials now support tissue preservation and promote remineralisation, extending conservative treatment possibilities even in deep caries.

Debate persists regarding the best approach to managing bacteria within infected dentine. However, improved insight into the interaction between oral microbial ecosystems and host tissues has informed more targeted and predictable strategies for deep lesion management, enabling reduced removal of sound tooth structure.

This presentation will provide an integrated overview of MI principles, highlight clinical techniques for early intervention and biological caries control, and outline the materials and decision-making frameworks essential for applying minimal intervention dentistry in modern practice.

Currently, he is the Vice Head of the School of Dentistry, Hanoi Medical University, as well as the Head of the Department of Pediatric Dentistry, and the Deputy Director of the Center for High-Tech Dentistry, president of Vietnam association of pediatric dentistry.

He graduated DDS in 2000, resident doctor in 2004 and a Doctor of Philosophy (PhD) in 2010, at Hanoi Medical University. He was recognized by the Vietnam State Council for Professor as Associate Professor in 2015 and Professor in 2022.

During his professional work, he has participated and chaired several research projects at Ministerial and national level. He has also published more than 160 scientific articles, including over 50 articles in reputable international journals in the ISI or Scopus repository. Furthermore, he has edited and contributed to the compilation of 18 textbooks, monographs and reference books for training and research.

Fluoride has long been established as the cornerstone of caries prevention; however, in contemporary dentistry, its role has evolved into a central tool for managing a broad spectrum of hard tissue pathologies. This presentation aims to redefine fluoride therapy through a clinical and individualized approach. Beyond the prevention and remineralization of incipient carious lesions, fluoride plays a pivotal role in treating dentin hypersensitivity, alleviating post-scaling discomfort in cases of cervical and root exposure, and managing developmental enamel defects such as Molar-Incisor Hypomineralization (MIH), enamel cracks, and other white spot lesions. Furthermore, this session will provide updates on novel fluoride formulations and discuss optimal concentrations for current clinical practice. Consequently, a practical framework for the effective, safe, and appropriate application of fluoride within the scope of minimal intervention dentistry will be proposed.

Background and Objective: Computer Assisted Implant Surgery (CAIS) has become an established standard for precise and accurate placement of dental implants. Either as static, dynamic or robotic, CAIS represents a complete workflow starting with digital treatment planning, enabling accurate implant placement and supporting advanced immediate loading protocols.

Content: Static and dynamic CAIS are based on different core technologies and therefore present different indications, possibilities and limitations. Static CAIS is currently the most established clinical approach, while dynamic technologies are rapidly evolving and challenging current standards. Robotic CAIS has also progressed beyond proof of principle, with thousands of implants already placed using robotic systems. Despite the rapid development of these technologies, broader implementation into routine clinical practice still faces several challenges, including high investment costs, learning curve, technical limitations, cost-effectiveness considerations, procedural scope and patient-related factors. This presentation discusses the current potential and limitations of different CAIS technologies through both clinical experience and scientific evidence. Recent studies and important aspects of static and dynamic systems beyond surgical accuracy will also be reviewed.

Conclusion: The presentation aims to provide insight into the future direction of CAIS and evaluate whether robotic implant surgery may become an essential component of modern implant practice.

Emeritus Professor Lakshman `Sam` Samaranayake of The University of Hing Kong is a renowned clinical academic with expertise in clinical microbiology and executive level administration. The author of over many texts, and 450 scientific communications – cited over 34,000 occasions (h-index 100), Professor Samaranayake has served over 13 years as the Executive Dean of dental schools in Hong Kong, Australia, and UAE. He has received numerous accolades for his exceptional contributions to dentistry, including the King James IV Professorship of the Royal College of Surgeons of Edinburgh, UK, and the Distinguished Scientist Award of IADR, USA. He holds many visiting/adjunct professoriate appointments in the universities of Thailand, Australia, UK, Indonesia, Sri Lanka, and China. From Biofilms to Data-Driven Diagnostics
Understanding of oral disease has evolved from a simplistic plaque-centric paradigm to a sophisticated model recognising dysbiotic biofilms as dynamic, host-modulated ecosystems. Dental biofilms are no longer viewed merely as accumulations of microorganisms but as complex, structured communities interacting with host immunity, environmental exposures, and systemic health. This ecological shift in thinking has laid the foundation for a broader diagnostic revolution.

Advances in microbiomics, metagenomics, and salivary biomarkers now enable detailed characterisation of biofilm composition, functional activity, and virulence potential. When integrated with artificial intelligence and big data analytics, these datasets allow risk stratification, early disease prediction, and personalised preventive strategies. Digital imaging, chairside biosensors, and longitudinal patient data further enhance our capacity to detect transitions from symbiosis to dysbiosis before irreversible tissue damage occurs. The exposome framework adds another critical dimension, linking biofilm behaviour to diet, lifestyle, socioeconomic determinants, and environmental exposures.

Together, these innovations transform biofilms from static pathological entities into measurable, predictive indicators within precision oral health models. This presentation examines how dysbiotic biofilms serve as both the biological driver and the analytical substrate of data-driven diagnostics, redefining oral pathology in the era of systems biology and digital intelligence.

– A.Prof. Meloni graduated in Dentistry with honors in 2003. He obtained a PhD in Preventive Odontostomatology in 2008 and a Master of Science in Oral Surgery in 2009. Barrier membrane selection is a key factor for predictable outcomes in Guided Bone Regeneration (GBR). Non-resorbable membranes such as PTFE provide excellent space maintenance but are associated with complications including exposure and the need for a second surgery. Thick cross-linked bovine pericardium membranes have been introduced as a resorbable alternative capable of combining mechanical stability with favorable biological behavior.

Recent clinical studies and case series have evaluated bovine pericardium membranes in socket preservation, periodontal regeneration, sinus augmentation and GBR procedures. Their multi-laminar collagen-elastin structure provides high tensile strength, good suturing resistance and prolonged resorption time when cross-linked. Clinical evidence shows predictable horizontal bone gain and moderate vertical augmentation when used with particulate grafts and membrane fixation. Compared with non-resorbable barriers, pericardium membranes demonstrate a more favorable healing profile and reduced complications in case of membrane exposure.
Conclusions
Cross-linked bovine pericardium membranes represent a reliable resorbable option for horizontal and moderate vertical bone augmentation, offering predictable outcomes while reducing patient morbidity and eliminating the need for membrane removal.

Severe maxillary bone resorption following complete tooth loss presents a major challenge in implant rehabilitation, particularly in cases of advanced atrophy. Two principal treatment strategies are currently employed: bone grafting to reconstruct the alveolar foundation and graftless approaches that maximize the use of residual bone. However, the optimal choice in specific clinical scenarios remains a subject of ongoing debate.

The objective of this presentation is to clarify the criteria for selecting between grafting and graftless approaches in the management of the edentulous maxilla, based on scientific evidence integrated with clinical experience.
This presentation synthesizes current literature and illustrates key concepts through representative clinical cases. Bone grafting enables prosthetically driven implant placement and is indicated in cases of moderate atrophy with high esthetic demands; however, it is associated with longer treatment time and increased risk of complications. In contrast, graftless solutions—such as tilted implants, pterygoid implants, and zygomatic implants—reduce treatment time and surgical invasiveness, making them particularly suitable for severe atrophy. These approaches, however, require advanced surgical expertise and careful management of technique-specific complications.

Treatment decisions should be guided by prosthetic objectives, anatomical conditions, and patient-related factors. Current trends emphasize personalized treatment planning, the integration of digital workflows, and minimally invasive strategies while maintaining long-term predictability.

AI makes many tasks in periodontal practice easier, alongside significant limitations. Periodontal interventions contribute to the environmental burden; however, periodontology can promote oral and systemic health, indirectly reducing carbon emissions from interventions.
Conclusion
Despite its many advantages, AI cannot replace clinicians, but it is a valuable supporting tool. On the other hand, periodontal practitioners also need to promote environmental protection and adopt sustainable oral health care practices.

In Japan, long-standing efforts to prevent dental caries and periodontal disease have led to substantial improvements in oral health. As a result, the “8020 Campaign” has achieved remarkable success, with more than 60% of older adults retaining 20 or more natural teeth at the age of 80 as of 2024. However, it has also become evident that even when the number of remaining teeth is maintained, a certain proportion of older adults experience difficulties in “eating and speaking” due to declines in oral functions such as mastication, swallowing, and articulation. This highlights that the goal of dental care has reached a stage where it should shift from simply maintaining the number of teeth to preserving and improving oral function.

Against this background, Japan has implemented several new strategies focusing on oral function. First is the concept of “oral frailty,” proposed as a population-based approach. This concept identifies the accumulation of subtle declines in oral function as an early warning sign of future frailty. Initiated by the Kanagawa Dental Association, it has been widely disseminated and socially implemented through collaboration with the Japan Dental Association and related academic societies. Second is “oral hypofunction,” a diagnostic category introduced into the national dental insurance system in 2018. This globally unique system enables objective assessment, diagnosis, and intervention for oral function, thereby formally positioning functional management within clinical dental practice. Third is the “oral function improvement service” under the long-term care insurance system, which focuses on prevention and targets older adults requiring care.

This lecture will provide an overview of these three key strategies and discuss the current status and significance of oral function management for older adults in Japan. It is my sincere hope that Japan’s experience will offer valuable insights for our colleagues in Vietnam, where population aging is rapidly progressing.

Class II malocclusion is one of the most prevalent orthodontic anomalies, typically characterized by a retrusive mandible, a protrusive maxilla, or a combination of both. Management ranges across all age groups and requires careful spatial analysis and customization.

In Class II malocclusion, the most prevalent component is retrusive lower jaw, frequently driven by mandibular retrognathism. Managing these cases efficiently requires appliances that minimize reliance on patient compliance while achieving stable skeletal and dental changes.

The Class II corrector has to be preferably hybrid so as to have the combination of rigidity and flexibility to offer sufficient biological response along with ease of functionality to the mastication and other functions. This makes the growth modulation predictable for the clinician and comfortable for the patient. FORSUS Fatigue Resistant Device (FRD) is a hybrid, semi-rigid, telescoping functional appliance which satisfies the necessary requirements of a Class II corrector.

The FORSUS Fixed Functional Appliance serves as a highly reliable, compliance-independent tool for Class II correction. It successfully bridges the gap between complex removable appliances and invasive surgical interventions for a wide range of growing and late-adolescent patients.

Clinical data demonstrates that the Forsus appliance effectively corrects Class II discrepancies through a combination of dental and skeletal effects. Key outcomes include successful distal movement of the maxillary dentition, mesial migration of the mandibular dentition, modulation of mandibular growth and modest restriction of maxillary forward growth.
